The volume of the facial cranial bone decreases on average by approx. 230 cm³ from the third to the sixth decade of life; in addition to the loss of collagen fibres, elastin and hyaluron, this leads to a sinking of the cheeks from a youthful V to an ageing A. This loss of filling can also be rejuvenated in the long-term again as needed and precisely, all by injections with autologous fatty tissue into the forehead, cheek, upper and lower jaw regions.
The stem cells contained in the fatty tissue are “all-rounders” and, on the one hand, allow blood vessels to sprout, so that about half to two-thirds of the freely transplanted fatty tissue heals on a long-term basis. In addition, these omnipotent stem cells recognise that they have been injected into the skin and lead to a true reverse-ageing of the skin through the formation of new collagen and elastin.
The body’s own fatty tissue is very well-suited for wrinkle treatment because it finds itself accepted particularly well by the body. The fat is taken from areas of the body with surplus tissue padding, such as areas of the abdominal wall, waist, hips or the inside of the knee area.

WHICH AREAS OF THE FACE CAN BE TREATED WITH AUTOLOGOUS FAT?
The following regions are particularly suitable for creating volume:
- Forehead: this also achieves a brow lift
- Temples: the emaciated concavity is given a harmonious transition from the forehead to the zygomatic arches.
- Zygomatic arches: Volume, which only the act of laughing could really enhance, and which could be seen in photos from 10 years ago (even without laughing), is once again visible on a long-term basis as a youthful V on the upper cheeks
- Tear troughs: here, the stem cells from the fatty tissue can even lead to a regeneration of lymphatic channels and thus, unlike hyaluronic acid, prevent water retention on a long-term basis.

WHAT ARE THE ADVANTAGES OF AUTOLOGOUS FAT TREATMENT IN THE FACE COMPARED WITH USING OTHER METHODS?
Autologous fat corrects volume loss (bone loss) and age-related skin quality (elastin, collagen, etc.) “scar-free”. This treats vertical ageing (from top to bottom) physiologically – unlike lifting procedures that (distort) the skin horizontally.
On the one hand, the body’s own stem cells allow blood vessels to sprout so that the freely transplanted fatty tissue can integrate in the long-term. In addition, the body’s own bioactive molecules coordinate skin rejuvenation by regenerating the ecosystem of the highly complex cell architecture. In summary, stem cells from adipose tissue accelerate collagen and elastin synthesis, support the skin’s water balance, as well as metabolic and anabolic processes and thus lead to a true reverse-ageing effect.
WHAT IS THE PROCEDURE FOR INJECTING AUTOLOGOUS FAT INTO THE FACE?
After liposuction, the fatty tissue is processed as microfat and nanofat at the operating table – i.e. during the operation. The body’s own material is inserted via cannulas so that there are virtually no scars.
FROM WHERE IS THE FAT FOR THE AUTOLOGOUS FAT TREATMENT TAKEN?
Fat tissue is suctioned from surplus padding that bothers the patient. Not only is as much fatty tissue removed as is necessary for facial rejuvenation, but liposuction is performed in the same way as aesthetic liposuction.
WHAT ARE THE RISKS OF LIPOFILLING?
With autologous fat transplantation into the face, there are the usual risks, as with all other operations: bruising, swelling, postoperative bleeding, injury to vessels and nerves, etc. It is, therefore, important that the surgeon is experienced and possesses a good knowledge of human anatomy. The primary limitation is the restriction of the ability to socialise for about two weeks.
WHAT DO YOU HAVE TO BEAR IN MIND AFTER WRINKLE TREATMENT WITH AUTOLOGOUS FAT?
A compression girdle should be worn at the donor site for 6 weeks. In the first few days, “inflammatory” wound healing takes place. Therefore, anti-inflammatories – such as aspirin, ibuprofen, and paracetamol – should not be taken during this period. Other painkillers are available if needed.
Cooling with ice should be avoided to facilitate the healing of the fat cells.
HOW LONG DO THE RESULTS LAST AFTER A WRINKLE INJECTION WITH AUTOLOGOUS FAT?
The general rule of thumb is that an autologous fat transplant, in which about 120 cc of volume is transplanted, will rejuvenate you by about 10 – sometimes 15 – years. Given that ageing processes progress, you can assume that, after about 10 years, you will look more or less as you did before the operation.

Reverse ageing takes place by building up the volume that has been reduced mainly through bone loss, and by regenerating elastin and collagen in the skin.
The procedure of removal, preparation and insertion of autologous fat tissue takes about 90 minutes.
Essentially, both techniques can achieve a rejuvenation effect of about 10-15 years. The cheek-neck lift tightens the skin in the areas mentioned. Autologous fat grafting restores previously lost volume to the face, and rejuvenates the skin through elastin and collagen regeneration, which cannot be achieved by a face-lift in any case – and is virtually scar-free!
Ultimately, in addition to the individual healing processes, the surgeon’s experience with this technique is also very important. If you “overfill” the findings accordingly, and thus take into account how the result will develop after the non-healed fat has been removed about 6-8 weeks on a postoperative basis, one round of treatment is sufficient.
Yes! When fatty tissue is treated with gentle care (avoiding local anaesthesia and centrifugation), up to two-thirds of the freely transplanted fatty tissue can integrate in the long-term.
About half to two-thirds of the freely transplanted fat tissue can integrate in the long-term. After about 6 weeks, the part that does not remain permanently and begins to break down.
The procedure can be performed under general anaesthesia. The removal of the patient’s own fat can also be performed under tumescent local anaesthesia. The second part of the treatment – the fat injection – can then be carried out under local anaesthetic followed by a brief “twilight sleep”.
The fatty tissue is inserted into the face via cannulas and leaves virtually no scars here. Small incisions of approx. 1 cm must be made in the donor area to remove the fatty tissue by suction.
After an autologous fat treatment in the facial area, your social calendar will be interrupted for about 2 weeks.
No cream or make-up should be applied for a week, to allow the freely transplanted cells to integrate.
After an autologous fat injection, there is usually a little swelling. Bruising may also occur. Infections, on the other hand, are rather rare!
Since it is the body’s own tissue, defensive reactions by the body’s immune system are unlikely.
